Incident Report and Resolution Summary

Overview
This afternoon we identified an issue that was preventing most G2G transactions from processing on the agency side.

What Happened
Beginning the evening of Sunday, June 21, 2026, APEX processes that handle SFTP file pickup and delivery were unable to access the SFTP folder. This occurred because our legacy SFTP server was shut down over the weekend, but some components were still attempting to use it. As a result:

  • Files sent by SFTP users (including all state agencies and many TTCs) were not delivered into APEX for processing.
  • Response files generated by counties were not sent back to the SFTP server for pickup.

Impact
From Sunday evening until this afternoon, outgoing and incoming G2G transaction files became stuck in the system and did not move through their normal processing flow.

Resolution
The issue was fully resolved today (6/24/26) at 3:20 PM, when the configuration pointing to the retired SFTP server was updated and verified. All affected processes resumed immediately, and file movement returned to normal.

Current Status
All systems are functioning as expected, and normal processing has been restored. We continue to monitor system performance to ensure stability.
